It has been a while since I installed this. There is no diagram and I got it mixed the first time. I believe there are three terminals: two short and one long. I believe the long is the ground. It is the two short ones that are confusing. These are hard to find outside a kit.

It is for a 1/4 inch male jack which is basically a guitar cord. It is soldiered to the lead lines + and - to a pickup of what ever kind you have whether it be under saddle or blended with microphone, etc. Works really well.
